She’s just had enough lipo to keep her waist at an ‘acceptable’ size, and never actually developed the self-discipline required to stop stuffing her face. She probably hasn’t done the research to find out that you can’t out exercise a bad diet either, and with legs like those, running’s going to be a struggle anyway... That said the whole exercise thing was a ruse. She was already using lipo to keep her stomach flat while her thighs and hips kept getting bigger as she got heavier. She hasn’t been able to get the lipo and as with most lipo procedures, the fat that sucked out will end up coming back but in a different area which is why her face back and neck are far now. The fact her face and neck are getting bigger is a good thing and the rest is slowly catching up aswell, she just has to grow bigger. If anything she'd look awful if she lost all the Weight because that'll probably be all done with surgeries and Very little excersize.
